Saint of the Slums is a Quest for Dragon's Dogma 2 (DD2). Saint of the Slums is affected by the passage of time. The Arisen will be compelled to find out the truth about the patients of The Gracious Hand under the care of Elena after hearing Vlasiy worry about his son's health.
The slums of Vernworth are a harsh place, where illness is oft synonymous with death. Yet amidst the despair, the abbess of the Gracious Hand stands out as a beacon of hope.
How to Unlock Saint of the Slums Quest in Dragon's Dogma 2
Outside The Gracious Hand, you will come across a father named Vlasiy complaining about his son's deteriorating health. Listen to him talk to Elena, then after he walks away, speak to Elena to trigger this quest. See Saint of the Slums on the [Dragon's Dogma 2 Map].

Give Elena the medicinal ingredient.
After Vlasiy and Elena's conversation, Elena will ask you to provide the Miasmite, a medicinal ingredient she intends to use on her patients. A miasmite can be harvested from phantoms, monsters that wander the world at night. If you have some stashed in one of the Inns, fetch them, and take them to Elena. Otherwise, you would need to explore at night and look for these phantoms to get 3 Miasmite.
With the Miasmite in her hands, Elena will invite you to join her in her rounds. Doing so will continue the progress of the Saint of the Slums and you will be tasked to follow Lady Elena. She will first take you to Lubomir, the son of Vlasiy who was heard earlier. The ladies of the Gracious Hand will now take their leave to prepare the medicine. Once it's just you in the room, Lubomir tells you that something is off about the place and will change your objective to "Investigate the Gracious Hand".
Investigate the Gracious Hand
Lubomir said that something seemed to be amiss with The Gracious Hand, after all, he started feeling weaker as the day went by. Your investigation will progress after speaking to a lady called Lottie, she is found in front of The Gracious Hand, cleaning the premises. After this, return to Lubomir. He will ask you to investigate further and go to the basement to find out what's happening. Accept his request to progress the quest.
Your objective changes to gather evidence and the entrance to the basement is on the right side of the building if you're coming from Lubomir's bed (you can only access the basement at night or the door will be locked). Head down the stairs and proceed to the door on the left. You will see rows of beds inside and enter another room with a curtain at the far end. Go inside and take two items: Records of Treatment from the bookcase and the Unlabeled Medicine on the medicine shelf. With the evidence in your hand, continue investigating by looking for the recovered patients treated by Lady Elena. The quest objective will update after picking up both items and you'll notice that the map will point you to the next location you need to investigate. Head to Walter's Tavern and look for Bruno and Jehan. They will tell you about how they wavered between life and death, and they adore her deeply for the care that she has given them.
NOTE: Some situations will make Elena flee when she spots you during her secret meeting but she didn't seem to mind when she saw us. But make sure when you're near or you spot them, approach the spot carefully and a dialogue cutscene will trigger.
Outcome 1: Apprehend Elena with enough Evidence
Continue your investigation by heading to the meeting spot of Elena and her contact at night. Her location will be tracked and you find out that she's meeting with Alois who's trading for Lord Phaesus. After the cutscene, head back to Lubomir and show him the evidence you've found and he'll mention a doctor that you can speak to at Checkpoint Rest Town.
You can travel there by foot or take the Oxcart to shorten the trip. You will find Radcliff in the Southeastern area of the town by going up the dirt path. His house is on the right side and can be accessed by going through the door next to the stairs. Give the Unlabeled Medicine and he will examine it for you, which will take just a day. Check on the doctor immediately to learn about his examination.
Radcliff informs you that it possesses addictive properties that will likely cause the patient to become dependent on the deadly effects. After learning about this "medicine", immediately return to The Gracious Hand to apprehend Elena. When you arrive, you'll notice that Lubomir is no longer in his bed, and Lottie, the one standing next to where Lubomir usually was will inform you that he's been moved to the basement since his condition has worsened. Now, when you reach the basement, you'll find Elena. All you have to do here is to grab and pin her down, a short scene will play and you will be taken to
Vernworth Castle Gaol Tower.
Since you have enough evidence and have proven that the medicine is a sham one of the guards will advise you of Elena's fate. This will complete the quest and acquire the following rewards: 1,600 XP, 0 Dcp, and 11,000 G,
Hidden Objective Post-End Of The quest
After the quest is finished (set as "Completed") you can, if chosen, talk to Lottie outside of The Gracious Hand. You will hear that she does not have enough hands to take care of everyone. You can take an Oxcart to Checkpoint Rest Town again to see Radcliff. He will ask you 3,000 G for him to go to The Gracious Hand. Doing this will save Lubomir life and Lottie will be pleased, she will reward you with 10 Blighting Arrows. Be careful, you need to do this right after the quest ends, as Lubomir can (and will) die if you do not visit Radcliff fast enough after the quest has ended.

She was the last person anyone would have expected to be engaged in secret meetings with suspicious characters after dark, but when you caught her in the act, she fled the scene. Lady Elena has since disappeared from Vernworth, leaving you with no way of finding out what she was up to.
You apprehended Elena but did not provide sufficient evidence:
The abbess has earned herself a reputation as "the lady saint" for her work caring for the ill of the slums—but you have good reason to believe that she is not so virtuous as she appears. However, while you saw to Lady Elena's arrest, the evidence you provided may not suffice to bring her misdeeds to light.

A reputation not entirely deserved, for in truth she has been poisoning a select portion of her patients in order to gain the favor of Lord Phaesus. Dragon's Dogma 2 Saint of the Slums Quest Notes & Tips
- This quest does not unlock any other quest
- This quest requires you to listen to the conversation between Vlasiy and Elena to trigger the quest.
- Lubomir can die here if you ignore the time. We recommend that you focus on completing this quest once you start it.
- There seems to be a bug that if you acquire all the evidence and head to the meeting spot of Elena and Alois, she won't react or just stand there after the dialogue cutscene. Some have mentioned you can just tackle her, but we highly recommend that Radcliff has finished examining the medicine so it's certified to be a toxic medication and will add to your evidence when you're sent to jail. Remember, that you need enough evidence before you apprehend Elena to prove her actions are ill-advised.

- After completing this quest, you can speak to Elena in the Vernworth Castle Gaol Tower to get more information on her motives. Wear the marcher's guard uniform to avoid being attacked by the prison guards. She explains that she was obtaining a rare substance from the bodies of the sick for important research being done at the Forbidden Magick Research Facility, and suggests you go there to learn more. As justification, she explains her belief in fate, and that the poor people in the slums are fated to die from sickness. As such, she might as well harvest this rare substance from them while they die so that their lives serve a higher purpose.

So, Lubomir vanished after I witnessed the meeting. Exactly how small of a time frame do you have? I did this as soon I got the quest and the only "extra" thing I did was pass the time to night.

Why do you tackle Elena instead of telling the guards? Why is there no quest objective for said task? Why can't you use a wakestone on Lubomirs' body? Why doesn't it appear in the Charnel House? Why can't you just tell Radcliffe to go to Vernworth when he tests the medicine? Why doesn't Lottie give a clue on where to find the abess' replacement after the quest?

Damn, was hoping I'd see some quest improvement in the sequel, but it's still as weird and nonsensical as ever.

Even after you get all the "evidence" the quest log still says "Gather evidence and apprehend the abbess.", with the more detailed entry being "Investigate the basement sickroom for evidence to implicate the abbess, then hand her over to the sentinels". So here I am, still searching for evidence, trying to talk to every guard I can find to have them arrest her, talking to the Captain. Nothing. I finally give up and look up a guide and you're just supposed to tackle her at this point? Bloody zero indication you're done and zero indication the next step is to ASSAULT HER. There is zero reason to believe you have the authority to conduct the arrest as in the eyes of the law at this point you're an ordinary citizen. Bad ending to an ok quest.

Dangitt. I tackled right after her nefarious midnight meeting ...she gets arrested but not enuff evidence...end of quest.. got nuffin ...and lobomir may be ded as he ain't talking now ;(

You don't get arrested after tackling her but instead a black screen happens and next you are outside the gaol where she was taken. There was never a conversation or video of the Arisen ever getting arrested, just: thank you for your efforts on this matter! From the guard.

cool bug i found is if the guy wont spawn for the night time meeting if you tackle the lady it will skip ahead past all the doctor stuff and make the quest completed. For some reason the guy she was supposed to meet never spawned so i just tackled her out of frustration and it immediately skipped the rest of the quest and gave me the rewards. cool game :D

Interestingly enough - if you did all the parts fast enough, you can still go the save Lubomir route. BUT: You need to talk to the concerned sister at the hospital first ! Her dialogue triggered the option to ask Radcliffe for his help (for 3000 g...). Before that I could not ask him, but it still worked after several days after finishing the quest.

Why isn't the option of saving lubomir listed? If you do the quest in the correct order without wasting time, then you can convince Radcliffe to leave the border town and come give lubomir some real medicine.
Lubomir gets saved and the doctor stays to run the place.

I got to the point of looking for evidence in the sick room and couldn't progress because the Records of Treatment doesnt show up on the bookshelf, so I'm permanently unable to do this quest.

You can visit Elena in the dungeon after getting all of the evidence and apprehending her. She reveals she was harvesting a resource from members of the slum as they were fated to waste away anyway.
You can free her if you have a key (and she will run away), though I am unsure what happens once you do so.

After you complete the quest, go back to the Checkpoint Rest Town and speak to Radcliff again, you'll tell him the abbess has been arrested and he'll ask for 3000 gold to attend to the sick in her stead. Afterwards go back to the slums, Lubomir and the other sister will be waiting out side for you, you'll be giving 10 blighting arrows.
Talking to Elena in her cell shows how much of a sociopath she is. She's one of the most evil NPCs in the entire game. It's pretty disturbing how she justifies giving innocent people living in the slum a slow and painful death and then demands that you praise her for it.
