If you make a forgery of it, using the fake will highlight all the stones in a very huge area on your map, but the second you close your map they all disappear and the forgery breaks. The original is infinite use, only marks in your general vicinity, and the markers stay on your map until you travel far enough away from that area.

One found at the top of the palace guard tower. Tower stairs are near Sven's room, if you take the stairs there are some jumps you'll have to do as stairs are missing in some parts.

Also in the tower are some ok early game armor (chainmail, sentry helmet) and a lantern

The tutorial pop up of this item says that the number of shards available will change over time? What exactly does this mean?

There is one in a chest to the west of the Riftstone of Remembrance/Bay Wayside Shrine area. The village part has a chest with a locked door that you need to spring somebody up into.
