I’m guessing Isaac was meant to sell this if you don’t give him the real on the transference of souls and he stays a vendor, but it either glitches his inventory or they cut it.

This might be a npc only item. Isaac wears it after you complete the first objective in his quest "Short-Sighted Ambition". I tried to see if I could pilfer it from him, I got a knockoff wokestone shard.
